Todd Nighswonger of the Neosho Daily News takes an in-depth look at a new initiative at ACCESS Family Care to help diabetic patients who are discharged from the hospital to better manage their condition. ACCESS will assign a community resource coordinator (CRC) to serve as a liaison with local hospitals to identify discharged patients with poorly controlled diabetes and connect them to services and educational opportunities, with the goal of reducing unnecessary ER visits and hospital admissions.
