Stronger Teams, Healthier Communities

The Association of Clinicians for the Underserved (ACU) held its Annual Conference July 28- 31, 2019 in Washington DC. The Conference focuses on bringing clinical leaders of all types together to focus on improving care for the underserved.

RCHN CHF population health program colleagues presented a workshop, “Health Center Approaches to Team-Based Care for Complex Patients”, focused on how community health centers are meeting the challenges of caring for individuals with complex needs. The workshop was moderated by Dr. David M. Stevens, and presented by Lakisha Samuels, of the Georgia Primary Care Association, Dana Longobardi and Kellan McNally of Fenway Health.
