Impact.

Since its founding in 2005, the RCHN Community Health Foundation has developed a wide range of projects and collaborated with community health centers, primary care associations, and public health institutions across the country to support community health centers and improve access to health care for the communities they serve.

RCHN CHF has designed and implemented several unique grant programs including the Population Health Initiative, Workforce Initiative, and the Outreach and Enrollment programs. Additionally, the Foundation has provided direct support through emergency grants, technical assistance and other investments to support health center transformation in the areas of technology and HIT support, operations, and education. A unique partnership with the Milken Institute School of Public Health at the George Washington Health University supports the Geiger Gibson/RCHN CHF Research Collaborative and our signature policy brief series.

“By the Numbers” showcases the impact and reach of our diverse work as the nation’s only foundation dedicated to advancing the community health center movement.
