RCHN CHF is a recognized leader in the health center movement, supporting innovative projects that benefit our nation’s health centers and underserved communities. Since its inception, the RCHN Community Health Foundation has provided $12 million in direct funding to support critical health center projects all over the country, through more than 40 grants and gifts. Program grantees in the Foundation’s Outreach and Enrollment initiative enrolled 31,000 people in coverage in one year alone, while workforce grantees have trained and supported hundreds of entry-level, front line staff. New Population Health projects are addressing disparities in six states through a collaborative learning approach. In addition, the Foundation’s broad-reaching educational, research and policy collaborations have produced more than four dozen white papers and special reports, along with an extensive body of significant peer-reviewed literature and special studies that are relied on by federal and state policy makers, the news media, as well as health centers and primary care associations across the country. In the words of one grantee, the Foundation is “more a partner than a funder.”

RCHN CHF has as its sole focus support of community health centers, which today form the single largest comprehensive primary health care system serving the nation’s most vulnerable medically underserved communities, and the backbone of America’s safety net. Today, health centers are the medical home to 25 million people, serving one in 14 individuals in urban and rural areas across the country. Yet with a rapidly evolving health care landscape, much remains to be done if health centers are to fulfill their mission of reducing disparities, improving health equity, lowering costs, and expanding access. Health centers need to adapt effectively to a new environment while maintaining the flexibility to address the needs of their complex and medically vulnerable community members.

Building on distinctive competencies and long-standing relationships, RCHN CHF is uniquely positioned to support community health centers by:

  • Anticipating industry trends and providing an extensive evidence base to help health centers remain at the forefront as health care policies evolve;
  • Partnering with community health centers and primary care associations around the nation to identify program needs, inform strategy and define future initiatives;
  • Capitalizing on the talents of a strong, experienced and agile staff and committed Board of Directors to provide thoughtful leadership that informs development of its signature health care programs;
  • Serving as a knowledgeable convener and catalyst to continue to disseminate best practices and promote health center development.
